By signing this waiver, I, the undersigned, acknowledge and agree to the following terms and conditions as a participant in Parents In Motion programs, events, workshops, and activities ("Activities").
1. Assumption of Risk. I understand that participation in the Activities may involve certain risks, including but not limited to physical injury, emotional distress, or other harm. I voluntarily assume all such risks.
2. Release of Liability. I hereby release, waive, and discharge Parents In Motion, its founders, officers, volunteers, and agents from any and all claims, demands, or causes of action arising out of my participation in the Activities.
3. Indemnification. I agree to indemnify and hold harmless Parents In Motion from any loss, liability, damage, or cost incurred due to my participation.
4. Media Release. I grant Parents In Motion permission to use photographs or recordings taken during Activities for educational and promotional purposes, unless I indicate otherwise in the notes field below.
5. Accuracy of Information. I certify that all information provided in this form is accurate and complete to the best of my knowledge.
6. Governing Law. This waiver shall be governed by the laws of the State of Illinois.
This waiver is legally binding. Please read it carefully before signing.
